Protective (neutropenic) isolation shields a severely immunocompromised patient FROM the environment, the reverse of standard isolation which protects others from the patient. It is used for very low neutrophil counts, such as after chemotherapy or stem cell transplant. The guiding principle is keeping organisms away from a patient who cannot fight infection.

What are the warning signs of Protective (Neutropenic) Isolation a nurse must report?

Report any temperature of 100.4 F (38 C) or higher at once as a possible neutropenic emergency. Never allow live plants, fresh flowers, or ill visitors into the room. Avoid invasive procedures and rectal temperatures or suppositories that breach barriers.

What do I need to know about Protective (Neutropenic) Isolation for the NCLEX?

Indicated when the absolute neutrophil count is low, generally below 1,000 and especially below 500 cells/mm3. A positive-pressure room with HEPA filtration helps keep airborne organisms out. No fresh flowers, standing water, or raw fruits and vegetables that can harbor bacteria or fungi. Strict hand hygiene by all who enter is the most important protective measure.
