The scenarioA 72-year-old is admitted from home with a 2-day history of dysuria and now confusion. Vitals: T 38.9 °C, HR 112, RR 24, BP 98/56, SpO₂ 93% on room air. Skin warm and flushed. WBC 16,000. Lactate 2.6 mmol/L. The family says "she was fine two days ago."

Step 1 — Recognize cues

Which findings should raise concern? Sort the signal from the noise:

Pattern recognition: infection source (UTI) + SIRS + organ dysfunction (confusion, hypotension, rising lactate) = sepsis, trending toward septic shock. Two quick screens: qSOFA (RR ≥ 22, altered mentation, SBP ≤ 100 — she hits all three) and the older SIRS criteria both flag her.

Step 2 — Analyze cues & prioritize hypotheses

Could this be simple dehydration or a UTI without sepsis? The confusion, hypotension, tachypnea, and elevated lactate push past an uncomplicated UTI. The priority hypothesis is sepsis from a urinary source, and the immediate threat is inadequate tissue perfusion. This is not a "monitor and reassess in an hour" patient.

Step 3 — Take action: the Hour-1 bundle

The Surviving Sepsis "Hour-1" bundle is exactly the kind of prioritized action set the NCLEX rewards. Initiate rapidly:

  1. Measure lactate (and re-measure if elevated).
  2. Obtain blood cultures before antibiotics — don't delay antibiotics long for it, but draw first when feasible.
  3. Administer broad-spectrum antibiotics.
  4. Begin rapid crystalloid fluids (e.g., 30 mL/kg) for hypotension or lactate ≥ 4.
  5. Apply vasopressors (norepinephrine is first-line) if the patient stays hypotensive despite fluids, to keep MAP ≥ 65 mmHg.
The high-yield NCLEX trapCultures come before antibiotics — but recognizing sepsis should never stall lifesaving treatment. If a question forces a single first action in a clearly deteriorating patient, prioritizing oxygenation/perfusion and antibiotics is defensible. Read each stem for what it's actually asking.

Step 4 — Evaluate outcomes

After the bundle, which findings show the patient is improving?

  • Improving: lactate trending down, MAP ≥ 65, urine output ≥ 0.5 mL/kg/hr, clearing mental status, HR normalizing.
  • Worsening (escalate): lactate rising, persistent hypotension despite fluids, falling urine output, worsening confusion, mottled skin (late "cold shock").
